Cavanaugh Distinguished Service Award

The Cavanaugh Distinguished Service Award recognizes an outstanding staff member who has contributed to the Arthur A. Dugoni School of Dentistry's vision, mission and values. The Cavanaugh Distinguished Service Award is funded by former University of the Pacific Vice President for Business and Finance Patrick Cavanaugh and his wife Janita. The winner of this award receives: 

A check in the amount of $500 
An award plaque 
A photo and feature article in the Pacific Insider

NOMINATIONS FOR THE CAVANAUGH DISTINGUISHED SERVICE AWARD

The nomination period for the Cavanaugh Distinguished Service Award begins October 6, 2014 and ends November 7, 2014. All members of the Pacific School of Dentistry community are encouraged to make nominations. Nomination forms must be completed for each nominee, and must be accompanied by 2 (no more than 3) letters of support from members of the Pacific community. A nomination form should not be used as a letter of support. Nominations for individuals must include specific examples that demonstrate:

o Contributions to the School of Dentistry's vision, mission and values
o Extraordinary service to the University
o Outstanding customer service
o Commitment to teamwork
o Other areas of excellence 

The nomination form is available below in the "Action Items" section. Completed nomination packets should be returned to the Human Resources office (4th floor, suite 406) by Friday, November 7th.  Nominations will be reviewed and a winner chosen by a selection committee made up of faculty and staff. The winner will be announced in early December.
