Absence Notification

Students are required to contact the Office of Academic Affairs promptly in case of absence from school so that timely notification goes to course directors and clinic personnel. The Attendance Policy is described in the School Catalog.

An excused absence is normally recognized when students contact the Office of Academic Affairs before 9:00 a.m. on the day of an absence or when other proper written notification has been received and approved.

Use one of the options listed here to notify Academic Affairs of your absence:

1. Submit a Request for Absence form (copies are also available in our office).
2. Call 415.929.6437 or 415.929.6436
   Voice mail is available Monday – Friday, 5:00 p.m. to 8:00 a.m. and weekends.
3. E-mail dentalabsence@pacific.edu
   Only emails from a dental school email account are acceptable.

All students requesting an absence should provide the following information:

• Name and student ID number
• Class year
• Date(s) and times of absence and date and time of return
• Reason for absence

Second- and third-year students should also provide the name of your GPA and indicate whether patients have been notified of your absence and whether you have a block or rotation on the day of your absence.
