All graduating students must receive an Authorization to Begin Closeout Process form from their Group Practice Leader (GPL). The authorization form is a checklist of items students must complete to clear from the school. Each student should return the form to the GPL, who will then inform the student when to pick up the closeout form from Duke Dahlin in the Business Office.
Each student must have completed the closeout process prior to commencement in order to receive a diploma.
Students will receive an email with a Qualtrics survey link to complete the Application to Participate in Commencement Ceremony in February 2014. Please use the Qualtrics link in the email; it is by invitation only and cannot be shared.
If you have a name that is hard to pronounce, please create an audio file with the correct pronunciation and email it as an attachment to Rachel Allyn-Crane at firstname.lastname@example.org by Friday, May 23, 2014.

You can order your regalia by clicking on Herff Jones. We will also include the link to order regalia in the email announcing the Qualtrics survey.
There is no charge to students to rent the academic regalia (cap, gown, hood, and tassel). Regalia can be purchased for approximately $508.95 for DDS students, $281.95 for hygiene students.
Academic regalia will be available for pick up at the following dates and times:
Tuesday, June 3, noon - 5:00 pm and Wednesday, June 4, 8:00 am - 2:00 pm

Students may invite their parents who are dentists or alumni of the Dugoni School of Dentistry to participate in their on-stage hooding as they walk the stage to receive their diploma. Each graduate receives his or her diploma alphabetically within the respective degree program. To ensure that candidates get the correct diploma and that the proper name is read, each candidate is assigned a specific seat. Seating charts will be posted on this site in advance of the ceremony. Students will be notified by email when the seating charts are posted.
The Commencement Ceremony will be held at the Orpheum Theatre, 1192 Market Street, San Francisco, on June 15, 2014 at 2:00 pm.
